UPDATE INSURANCE INFORMATION
If you ever need to update your insurance information, just fill out this HIPAA-compliant Insurance Information Form. Your response will go directly to Joanna and she’ll verify your benefits and update your account for you.
It’s so important to let us know when your insurance changes or when you are aware of pending change – ahead of time is ALWAYS better. If you don’t tell us, we have to rely on the insurance company to tell us and they often take their time. This can – and has – resulted in high balances that create stress and sometimes lead to pausing services for clients.
CANCEL / RESCHEDULE AN APPOINTMENT
To cancel or reschedule an appointment, you should always contact your clinician directly via email. Their email address will be [FirstName]@chosenpathcollective.com.
UPDATE PAYMENT INFORMATION
If you ever need to update your payment information on file, please reach out to Joanna at billing@chosenpathcollective.com and she’ll send the appropriate form to your client portal.
REQUESTING MEDICAL RECORDS
Your medical records belong to you, and we’re always happy to provide them in a digital format upon request. If you’d like us to send your records to someone else – like another provider or organization – we’ll just need you to complete a Release of Information (ROI) form first. You can request your medical records by sending an email to contact@chosenpathcollective.com.

TROUBLE WITH TELEHEALTH SESSIONS
If you’re having trouble joining a telehealth session, start by checking your internet connection and making sure you’re not on your phone or in a moving vehicle – telehealth works best with a strong, stable connection and a larger screen.
